Your Consent to Receive the Flu Vaccination

At Lindley Village Surgery, flu vaccinations may be given by either a doctor, nurse, nurse associate or a suitably trained Healthcare Assistant (HCA).

Before booking your flu vaccination appointment, please read the information provided about the flu vaccine:

By booking an appointment for your flu vaccination, you are confirming that:

• You have read and understood the information provided about the flu vaccine.
• You have had the opportunity to consider whether you wish to receive the vaccination.
• You consent to receiving the flu vaccination at your appointment.

If you have any questions about the vaccine, would like further information, or are unsure whether the vaccination is right for you, please do not use the booking link.

Instead, please contact the surgery and arrange an appointment with a GP or Practice Nurse. They will be happy to discuss the vaccination with you and answer any questions before you make your decision.

On the day of your appointment

The clinician administering the vaccine will confirm your identity, check that you are eligible for vaccination, check for any contraindications and make sure that you are still happy to proceed. Clinician may request you to rebook if necessary.
